Dental,Vision & Hearing
Dental
Our Dental plan, provided by Delta Dental of Iowa, is available to physicians and employees regularly working 20 or more hours per week. For additional details and plan design, view the Dental Overview, or contact Delta Dental of Iowa at www.deltadentalia.com or 1-800-544-0718.
Vision
Our Vision plan, provided by VSP, is available to physicians and employees regularly working 20 or more hours per week. For additional details and plan design, view the Vision Plan overview, or contact VSP at www.vsp.com or 1-800-877-7195.
Financial
Health Savings Account (HSA)
The Health Savings Account (HSA), is only available to HDHP plan participants. For 2026, the maximum HSA contribution amounts are $4,400 single/ $8,750 family. In addition, individuals who are age 55 or older can also make “catch-up” contributions.
The maximum annual catch-up contribution amount is $1,000.
Healthcare FSA
The Health Care FSA is provided by HealthEquity. The maximum 2026 Healthcare FSA Contribution is $3,400
*Physicians are not eligible for these benefits.
Dependent Care FSA
The Dependent Care FSA is provided by HealthEquity. The maximum 2026 Dependent Care FSA Contribution is $3,750 if married filing single; or $7,500 if married filing jointly or head of household.

Limited Purpose FSA
The Limited Purpose FSA is provided by Health Equity and is available only to those who enroll in the High-Deductible Health Plan with HSA. The maximum annual contribution limit for 2026 is $3,400.

Life, AD&D, & Disability Insurance
Life and AD&D
Our Life/AD&D plans, provided by Reliance Standard, are available to physicians and employees regularly working 32 or more hours per week. Our Long-Term Disability (LTD) plan, provided by Northwestern Mutual, is available to physicians and employees regularly working 32 or more hours per week.
